  • PSA Birth Certificate — original or certified true copy from the Philippine Statistics Authority
  • Certificate of Good Moral Character — issued by your previous school's principal or guidance counselor
  • 2 pcs. 2×2 ID Pictures — recent, white background, formal attire
  • Brown Envelopes — 2 pieces for document submission
  • Official Transcript of Records (OTR) — from your previous college or university
  • Honorable Dismissal — official document of honorable separation from previous institution
